Free Programmes: Free and Evidence-Based Weight Loss Programmes
You do not always need a private medical clinic to lose weight; free, self-managed programmes can be highly effective. If you are generally healthy, have no major weight-related complications, and do not require long-term medication, starting with a structured, free approach is entirely reasonable.
At Dr Bazire's clinic, we recommend exploring these first; just ensure you choose an evidence-based plan that supports your metabolic health rather than a fad diet.
The Best Free Options Available in the UK
The FreshWell App (Free)
Developed by NHS doctors. Endorsed by the Public Health Collaboration UK.
The FreshWell app offers a structured low-carbohydrate programme with clear dietary guidance, meal plans, and educational content. It is ideal method if you are just starting your weight management journey or looking to address early-stage insulin resistance without private support.
Intermittent Fasting Options
Intermittent fasting is term that includes several distinct approaches that vary according to the time during which you eat each day, or variations in the calorie intake on different days. For example:
The 5:2 Diet (for the cost of a book)
Popularised by the late Dr Michael Mosley.
Eat normally five days a week, then restrict intake to around 500–600 kcal on two non-consecutive days. There is solid clinical evidence supporting intermittent calorie restriction for weight loss and metabolic improvement — and it works particularly well for people who find daily calorie tracking hard to sustain. Dr Mosley's books are easy to follow and there are useful additional resources (recipes). All are widely available at modest cost.
Time-Restricted Eating (Free)
No app, no equipment, no cost — just a consistent daily schedule.
TRE means eating all your food within a defined window — typically 8 or 10 hours — and drinking only water or non-calorific drinks for the rest of the day. Evidence suggests it supports modest fat loss and improves blood sugar markers, especially by cutting out late-night snacking and reducing ultra-processed food intake.
NHS Weight Management Services (Free)
Clinically supervised support, at no cost.
The NHS has a free Weight Loss Plan delivered via an app that you can download here: https://www.nhs.uk/better-health/lose-weight/#download.
The NHS also commissions tiered weight management services with structured, clinician-led support — particularly valuable if you have a condition like type 2 diabetes or hypertension.
Note: Availability varies by region, eligibility criteria apply, and waiting times can be long.
Coming soon: The Elevate Health app by the Public Health Collaboration (subscription)
From the Public Health Collaboration (PHC UK) — a charity led by health professionals with special interest in helping people eat a healthy diet and overcome the challenges of the modern food environment.
PHC UK exists to promote healthy eating, weight control and metabolic health, and to raise awareness of the harms of ultra-processed foods and refined carbohydrates. Their Elevate Health app is currently undergoing beta testing and will soon be available to the public for a modest monthly subscription. The app offers in-depth diet and lifestyle guidance with progress tracking built in.

Who Are These Publicly Available Programmes Suitable For?
A Good Fit If You:
Have a BMI under 30 with no significant underlying medical conditions.
Are entirely new to structured weight management.
Want to establish healthier eating habits independently, without medical supervision.
Are highly motivated and have not previously struggled with weight regain or the "yo-yo" dieting cycle.
Consider Professional Support If You:
Have a BMI over 30 or struggle with severe obesity.
Have tried multiple self-managed approaches over the years without lasting success.
Have weight-related health conditions such as type 2 diabetes, high blood pressure, PCOS, or hypothyroidism.
Are considering medical weight loss options, such as GLP-1 medications (Wegovy or Mounjaro), which require strict multidisciplinary tracking to ensure healthy weight loss.
Frequently Asked Questions
If your general health is good, you have a BMI under 30, and you have not had repeated cycles of weight loss and weight regain. Dr Bazire recommends starting with evidence-based tools like the FreshWell app or the 5:2 approach. If these have not worked, or your medical history is complex, professional support will significantly improve your long-term health outcomes.
They offer reasonable evidence for short-term weight loss and peer motivation, but long-term effectiveness varies. Because they do not account for individual metabolic health, muscle preservation, or underlying endocrine issues, they cannot replace a medical assessment for chronic conditions.
Conditions like hypothyroidism and polycystic ovary syndrome (now renamed polyendocrine metabolic ovarian syndrome or PMOS) fundamentally alter hormone balance and metabolic rate. Free, self-managed plans are rarely sufficient to overcome these complex biological challenges, making a professional multidisciplinary assessment highly recommended.
Need Help Choosing Your Next Step?
Free programmes are a great start, but they can carry additional risks or lack efficacy if you struggle with medical complications, repeated weight regain, or emotional eating.
Book a Consultation: If biological barriers, underlying conditions, or past dieting attempts are limiting your weight loss, we can help you safely break the cycle using a structured medical approach.
