Weight loss surgery is reserved for patients with a BMI over 40, or over 35 in individuals with weight-related complications such as type 2 diabetes or high blood pressure, and in whom less invasive treatments have not achieved the required result. Surgery is never a standalone strategy; it works best as part of a structured, medically supervised programme that addresses diet, lifestyle, and long-term behaviour.

Surgical procedures range from temporary, minimally invasive options to permanent interventions. The most commonly used options, listed in order of increasing efficacy and risk, include:
Gastric Balloons:
A temporary, reversible option. A silicone balloon is placed in the stomach to induce fullness and reduce the hunger hormone ghrelin. Typically left in place for up to 6 months, with average weight loss of 10–15% of body weight.
Stomach Botox:
Botox injections delivered by endoscopy slow gastric emptying, increasing fullness for 4 to 6 months. A useful bridge while building healthier habits.
Gastric Band (Lap-Band):
A reversible silicone ring placed laparoscopically around the upper stomach. Adjustable via a port under the skin. Can achieve 20–25% weight loss and may be left in place long term.
Endoscopic Sleeve Gastrectomy:
A reversible technique that reduces stomach volume by ~80% using internal sutures, without surgical removal.
Laparoscopic Sleeve Gastrectomy:
An irreversible procedure removing ~80% of the stomach. Reduces both volume and ghrelin production. Long-term failure rates mean some patients later require revision surgery.
Gastric Bypass:
The most effective surgical option, dividing the stomach and rerouting the intestine to limit intake and absorption. Carries the highest risk but delivers the most significant metabolic change.
